Independent SEND Safeguarding & Wellbeing Audits

Guided Futures Advocacy provides independent safeguarding and wellbeing oversight through structured audit visits designed to observe, assess, and report on the safety, wellbeing, and support arrangements of vulnerable adults.

These visits provide professional, independent observations that help local authorities, care providers, supported living services, residential services, and other organisations identify emerging concerns early, strengthen safeguarding visibility, and promote safer, more positive outcomes.

Our audit visits are not intended to replace statutory safeguarding responsibilities. Instead, they provide an additional layer of independent observation, quality assurance, and preventative safeguarding support that complements existing safeguarding and quality monitoring processes.

Where concerns are identified, we encourage early intervention and provide practical recommendations to help reduce escalation risks, strengthen safeguarding practice, and improve outcomes for the individuals being supported.

Each Audit Visit Includes

Wellbeing Observation

Observation of the individual's emotional wellbeing, physical wellbeing, engagement with their environment, daily experiences, and interactions with those supporting them.

Safeguarding Indicator Review

Professional assessment of potential safeguarding indicators, including neglect, self-neglect, environmental concerns, changes in presentation, wellbeing risks, or other factors requiring further attention.

Environmental Safety Assessment

Observation of the individual's living environment and support setting to identify potential risks, safeguarding concerns, or factors that may impact safety, wellbeing, dignity, and quality of life.

Individual Voice Engagement

Where appropriate, practitioners seek to engage directly with the individual to better understand their experiences, wishes, feelings, preferences, and perspective on the support they receive.

Professional Written Report

A structured, evidence-based report outlining observations, identified risks, areas of good practice, professional recommendations, and suggested actions where appropriate.

Preventative Intervention & Follow-Up Support

Where concerns are identified, Guided Futures Advocacy provides practical recommendations, encourages early action, and may carry out follow-up oversight visits where appropriate to support positive improvement and reduce the likelihood of concerns escalating.

All reports are prepared objectively, professionally, and in accordance with safeguarding best practice, relevant legislation, and recognised safeguarding guidance.
